Register My Athlete:
- All athletes MUST register with Register My Athlete. No try out will be allowed unless registration is COMPLETE
Annual Physical Form
- HERE is the link to the document that will need to be taken to your physician when conducting a physical for the purposes of athletic eligibility.
- A physical is required each year as part of determining eligibility for all sports. This physical (and all associated registration steps through www.registermyathlete.com) MUST be completed before a prospective athlete is even allowed to try out for a sport. The document linked here is the only physical paperwork that can be used. Once that physical is completed, all pages, with required signatures, can be scanned and uploaded through www.registermyathlete.com or turned into the athletic office at Corner Canyon.
Banner Form
- The contract BELOW will need to be filled out by new and returning banner sponsors.
- Please email a copy of the completed form to [email protected] and turn in the hard copy, along with the sponsors check to the school office
- A portion of the banner cost will go to the school and a portion will cover the boys athletic fees.
- Please note, you may NOT call on any of the sponsors listed here unless they sponsored you in 2022. The athletes that secured those sponsors in 2022 have the opportunity to call on them in 2023
- Banners will be hung durning the lacrosse season on the East side of the stadium.
- All sponsors will be mentioned over the PA system durning home games.
